New Student Enrollment
For Students NEW to MMSD who live in the East attendance area (not from a private, parochial or charter school):
- Complete the online enrollment application at mmsd.org/enroll.
- Contact Lisi Daniels, East Registrar, by phone (608-204-1629) or email (aadaniels@madison.k12.wi.us) to set up a virtual appointment to verify the student’s enrollment and documents after completing the online enrollment application at mmsd.org/enroll.
To verify enrollment with the registrar, a parent will need to show the below documents via virtual meeting:
- Valid Photo ID
- Driver’s License
- Passport
- Government issued ID: Photo ID with full name and DOB
- Verification of Student’s Name and Date of Birth (ONE of the following):
- Original Birth Certificate
- Passport
- Health Record
- Proof of Residence (ONE of the following documents in the parent/guardian name):
- Lease
- Recent Utility Bill
- Mortgage
- Proof of Immunizations:
- Wisconsin State immunization records if access to Wisconsin Immunization Registry is not granted by parent
- All out-of-state immunization records
- Parent/guardian-student Relationship Verification:
- Verification of parent/guardian identity and relationship -- official photo identification (driver’s license, passport or government issued ID) and the student’s birth certificate or court documentation providing proof of parent/guardian relationship.
NOTE: The parent who enrolls the student confirms the verification of the second parent as long as the second parent's name is listed in the student's birth certificate.
NOTE: If the student received their education outside of the U.S., provide Academic History (any or all of the following):
- Last (most recent) report card (required if the student is an incoming 9th grade student. This is required to assist with scheduling)
- Special education information
- Withdrawal form
- Transcript
For incoming 9th grade students (Class of 2026) from a private, parochial or charter school who live in the East attendance area:
STEP 1 - MMSD Enrollment
1. Complete this Short App. (if student is a former MMSD student, skip short app and contact Lisi Daniels, East Registrar, by phone (608-204-1629) or email (aadaniels@madison.k12.wi.us) to set up a virtual appointment to verify the student’s enrollment and documents)
2. Once the short app has been completed, contact Lisi Daniels, East Registrar, by phone (608-204-1629) or email (aadaniels@madison.k12.wi.us) to set up a virtual appointment to verify the student’s enrollment and documents.

(Students whose parents complete the short app will then follow the instructions for "returning students" in August. They will also have to complete the regular online registration)
STEP 2 - Schedule Classes/Course Selection
- Review the 9th Grade Course Selection Card (Courses At-A-Glance)
- Review the EHS Welcome to Ninth Grade webpage for academic programming information
- Additional course selection content can be found on the EHS Counseling Website (course cards, sample course sequence, and course catalog)
- Submit the 9th Grade Course Selection form (once enrollment is completed with Lisi)
*Students can earn HS credit for 8th grade courses if they provide a HS transcript; 8th grade students are eligible for summer first time PE1.
STEP 3 - Final School Registration (AUGUST)
- Follow instructions for MMSD Registration for "Returning Students" in August to register students for the upcoming school year.
